Is there anywhere that states which medications are considered high-risk medication management to determine Medical Decision Making? I am specifically looking for CMS documentation.

Remember, the Table of Risk lists this as "Drug therapy requiring intensive monitoring for toxicity." Chemotherapy drugs requires this kind of monitoring.
