Wiki Hero Vascular Device

Hello, It has been awhile since I have coded for the "Hero Vascular Device" which if for renal failure. I can not find any info on how to code this. I know you normally use 36830 but there, I believe two other codes and I just can't remember or find any coding help.

Does anyone code for these?

Any assistance would be appreciated.!

I work for a vascular group and code this fairly often.

36830 & 36558 - together they make the Hero graft.
