PREOPERATIVE DIAGNOSIS: Bilateral inguinal hernias.

POSTOPERATIVE DIAGNOSIS: Bilateral indirect cord lipomas.

PROCEDURE PERFORMED: Laparoscopic bilateral inguinal hernia repairs with mesh (TEPP).

We turned our attention to the right side, where we carefully dissected out pubic tubercle as well as the indirect space. We found some direct space laxity but no direct hernia nor femoral. In the indirect space we identified vas deferens and gonadal vessels and swept the peritoneum medially. We found a moderate-size cord lipoma, which we reduced out of the inguinal canal completely.

We then turned our attention to the left side, again identifying superior pubic ramus. No overt direct hernia noted. No femoral hernia. We then dissected out in the indirect space and again found, as we reduced the peritoneum widely cephalad medially, a cord lipoma, which we reduced out of the inguinal canal completely.

For the above would the diagnosis be the cord lipoma only or is it considered a hernia because the lipoma was in the inguinal canal or do you code both?
