POSTOPERATIVE DIAGNOSES: Thrombosed external hemorrhoids and rectal polyp.
OPERATIONS: Excision of rectal polyp x2 and excision of thrombosed external hemorrhoids.
PROCEDURE:
The patient was taken to the OR, prepped and draped in the usual sterile fashion in Jackknife prone position, allowing us access with visualization of two polyps, one in the 3 o'clock position and one in the 9 o'clock position. had external hemorrhoids associated with one in the 9 o'clock position. We placed a 3-0 Vicryl stitch at the apex above the polyp, and using electrocautery, carved an elliptical excision excising the entire polyp in the 3 o'clock position. Using our previous stitch, we then did a running locking suture securing the wound shut. We paid attention to the polyp at the 9 o'clock position in a similar fashion ensuring that our elliptical incision excised the thrombosed external hemorrhoid on that area, and we closed it with the 3-0 Vicryl which had been previously placed at the apex of the wound. There was excellent hemostasis. All needle and sponge counts were correct.
se. Specimens were sent to Pathology for permanent section.
