Anyone willing to help code the following?

1. Ultrasound-guided access to right common femoral artery
2. Diagnostic aortogram including iliac arteries
3. Angioplasty and stenting of external iliac artery

Access to right common femoral artery with ultrasound guidance. Sheath inserted with ultra flush cath to the level of L1-L2. Diagnostic aortogram was performed visualizing the aorta as well as iliac vessels. The iliac artery appeared to have high-grade stenosis of greater than 90%. Decision was made to proceed with intervention. We then continued with a left lower angiogram for further evaluation of the vasculature. This showed SFA occlusion with occluded stents. The popliteal region was widely patent with flow through the anterior tibial and peroneal artery. At this point a glide wire was advanced into the level of the profunda femoris artery and a 6-French Ansell sheath was inserted under direct vision to the level of the iliac bifurcation and the catheter was placed in to the external iliac artery. A stent was deployed here and then angioplasty was performed. At completion
the wires and catheters were withdrawn into the aorta and a retrograde injection was performed from here. This idientified a complete occlusion of the extrenal iliac aretry extending all the way down the common femroal artery with flow through the profunda.

I'm sure of code 37221 but would it also be appropriate to code the diagnostic aortogram 75625 and the bilateral extremity angiograms?

With the report as it is, I would not code 76937- components of this code has to be met before reported.
If there are no diag exams prior to this intervention indicating stenosis (mra, cta etc.), then it is ok to code diagnostic imaging.

hope this helps...
