ELECTROPHYSIOLOGIC FINDINGS:
i. The left median sensory nerve conduction study at the index finger
demonstrated normal peak latency (2.9 ms) and normal sensory nerge action
potential amplitude (21.5 microvolts).
2. The left ulnar sensory nerve conduction study at the fifth finger
demonstrated normal peak latency (3.15 ms) and norma! sensory nerve action
potential amplitude (23.9 microvolts).
3. The right median sensory nerve conduction study at the index finger
demonstrated normal peak latency (3.1 ms) and normal sensory nerve action
potential amplitude (34.8 microvolts).
4. The right ulnar sensory nerve conduction study at the fifth finger
demonstrated normal peak latency (3.2 ms) and normal sensory nerve action
potential amplitude (21.1 microvolts).
5. Paired sensory nerve conduction studies of the median and ulnar nerves
at the left ring finger demonstrated normal peak latency, normal sensory nerve
action po%e~tial amplitudes and no significant differences in the peak
latencies between the two nerves.
6. Paired sensory nerve conduction studies of the median and radial nerves
at the left thumb demonstrated normal peak latencies, norma! sensory nerve
action jotential amplitudes, and no significant differences in the peak
latencies between the 2 nerves.
7. Paired transcarpal orthodromic sensory nerve conduction studies of theleft median and ulnar nerves at the palm demonstrated normal peak latencies,

8. The Combined Sensory Index of the left limb did not demonstrate a
significant slowing of the median nerve compared to the radial and ulnar
nerves.

14. The left, ulnar motor nerve conduction study demonstrated normal
distal latency (3.05 ms), normal compound muscle action potential
amplitudes (9.3 millivolts at the wrist, 8.7 millivolts at the below
elbow site, and 7.8 millivolts at the above elbow site close), and
normal conduction velocity across the elbow and through the forearm
(64.4 and 63.9 m/sec respectively).
15. The right median motor nerve conduction study demonstrated norma!
distal latency (3.45 ms), normal compound muscle action potential
amplitudes (8.7 millivolts at the wrist, 8.1 mV at the elbow site), and
normal conduction velocity through the forearm (57.3 m/sec).
16. The right ulnar motor nerve conduction study demonstrated normal
distal latency (2.8 ms), normal compound muscle action potential
amplitudes (9.9 millivolts at the wrist, i0 millivolts at the be!ow
elbow site, and 9.7 millivolts at the above elbow site), and normal
conduction velocity across the elbow and through the forearm (62.8 and
60.7 m/sec) .
Temperature recorded at the bilateral upper extremities was 33 degrees

My thoughts are 95904 X6 but the motor 95900 I am stuck on.
Any help is appreciated.
