How would you code the following? This was for Morton's Neuroma.

A alcohol injection consisting of 1.5 cc of a mixture of dehydrated alcohol and 1% Lidocain plain was infiltrated in 3rd interspace b/l around the symptomatic area.

Bilateral, you said?

64455 -RT and
64455 -LT

Unless it's Medicare FFS - and then I'd recommend 64455 -50 x1 with the fee doubled.
