A 62-year-old man has a 4 year old history of prostate cancer and is admitted through the ED with complaints of severe low back pain. He has been diagnosed with metastases to the testes and the lymph nodes in his groin. A recent bone scan showed a hot spot in the lower lumbar vertebrae. A lumber x-ray revealed a fracture of the L4 vertebrae. MRI revealed metastasis to the spinal cord and vertebrae at the L4 fracture site. He was started on MS continue to relieve the pain and transfer arrangements were made to a skilled nursing facility.

Please help me code this
 
You code the prostate cancer as history of, you code the metastatic sites using the neoplasm table look under the column for secondary, and you code a pathological fx of the vertebrae , the fx code would be first listed.
