Patient underwent revision right total knee and surgeon also performed bone grafting of osteolytic defects. I was looking at 27415 but there was no cartilage defect. Would you place mod 22 on 27487 for the revision? TIA
 
If allograft, it is included. Read the code description (either 27486 or 27487 depending) it states with or without allograft. If you don't have it and are doing a lot of ortho I would suggest obtaining the AAOS Global Service Data guide. It is really helpful as a reference for surgical coding. However, in this case it's in the CPT description. This does not warrant a 22 in my opinion from the brief info.
