We have a High Complexity clinic lab. Can we bill the ordering physician as the rendering or does the rendering have to go out as the director of the lab?

I think I can help you with this as I work for a high complexity reference lab. If you are meaning box 24J on the CMS 1500 form we enter our Taxonomy code on the upper shaded line and our NPI on the lower line. Hope this helps!
