A female came in for her annual well woman she had an exam, a vaginal pap smear along with an HPV screen & she's had a LAVH so would I code
v72.31 routine gynecological exam, AND
v73.81 since we also did an HVP screen, AND
v76.47 since she had a vaginal pap smear AND
v88.01 since she's has a LAVH??

Need clarification. Thx
