We are stuck on this one... we found code 76937 for U/S guidance but it's an add on code. We are not sure what to use for primary or if there is a better code out there???? Thank you for any help!!

PREOPERATIVE DIAGNOSIS:
1. Endstage renal failure.
2. Systemic lupus.
3. Inability to access AV fistula.

POSTOPERATIVE DIAGNOSIS:
1. Endstage renal failure.
2. Systemic lupus.
3. Inability to access AV fistula.

PROCEDURE PERFORMED:
Ultrasound guided access of AV fistula.

ANESTHESIA: None.

INDICATIONS, RISKS, BENEFITS AND OPTIONS: This is a XX-year-old
female who is in need of dialysis. I was contacted by the dialysis
nurses that they were unable to access the AV fistula even though
there was a great thrill. I attempted to access the AV fistula
without the use of ultrasound and was unsuccessful in doing so. At
this point, I discussed accessing the fistula under ultrasound with
the patient. The patient asked that we proceed on with performing
this procedure.

DESCRIPTION OF PROCEDURE: Sonosite ultrasound machine was then used
under sterile technique. The area was prepped and draped in usual
sterile fashion using Chlorhexidine prep and sterile drapes. The area
distal to the wound was then ultrasounded. The patient was noted to
have a good thrill. There was no clot seen within the fistula. I
accessed the fistula with the bevel side up visualizing under
ultrasound. We got good return. The needle was visualized in place.
The patient tolerated the procedure well.
 
Typically you would be doing a fistulogram to check for patency of the fistula. The op note is very brief and doesn't state if runoffs/dye were used to check the patency. If you did those, then 36147 would apply.
