Help? Humana Medicare PPO bundling - denying -XU modifier as not effective on DOS

Hello all. Humana is not honoring the -XU modifier appended to 95957 for DOS 5/31/19. Humana's reason is that the -XU was not effective on the DOS. I've checked the CMS website & our local MAC, & they say the -XU became effective on 1/1/15. I've also checked Humana.com & cannot find an effective date, just a definition of the modifier.
Has anyone else experienced this w/ Humana or any other carriers? Any feedback will be terrific. Thanks;)
 
I just had a similar denial for modifier 59 stating that the modifier is invalid for a June DOS. I was going to try an X modifier but after seeing your denial I'm starting to think this might be a system issue they are having. Let me know if you find anything out.
