I'm in Ohio with CGS Administrators for MAC Part B 15202. Their LCD L34409 for MBB and RFA states under frequency with critieria, "Two radiofrequency neurotomies per year involving 2 joints per region per session may be performed 2 times a year". Our physician, for example, will do L3,4,5 right (2 joints) and then a few weeks later, L3,4,5 left (2 more joints). If positive response, he will repeat in 6 months. Are we within the criteria? He does not repeat RF on the same 2 joints more than 2 times per year and he does not do more than 2 joints per session.

They don't seem to address the staged procedure for the contralateral side. If the patient is under moderate anesthesia care, it seems the bilateral procedure could be performed in one setting.
