Question: Is it appropriate to report code 27132 when the surgeon is converting a hemiarthroplasty to a total hip arthroplasty? The patient had a previous hemiarthroplasty, femoral component that was being revised/replaced with additional placement of the acetabular component. Answer: Yes, code 27132, Conversion of previous hip surgery to total hip arthroplasty, with or without autograft or allograft, applies to any previous hip surgery (except total hip arthroplasty) in the patient’s surgical history in which a skin incision was made. To illustrate, if the patient had a previous hip surgery (eg, fracture fixation, hip pinning for a slipped capital femoral epiphysis, or osteotomy), the performance of a hip surgery to convert the prior surgery to a total hip arthroplasty, it would be reported with code 27132. Removal of any old hardware is considered an intraprocedural component of code 27132 and, therefore, it would not be reported separately
