This patient had a bone cyst removed and a bone substitute was utilized after the craterization procedure. The bone substitute was meant to be permanent. 3 weeks later, the patient has developed a staph infection. "......The previous surgical incision was opened. Purulent material was noticed. This tracked down to the ulna where the bone graft was performed. Deep cultures were obtained. All bone graft material was removed. Over 6 liters of fluid, 3 with Bacitracin were used for irrigation. The incision was then closed over a Penrose drain." I am looking at CPT code 25145-78. I did not feel that 11044 was appropriate since OP note does not mention any type of debridement at all. Please advise.

After much research, I can only say that I would code an Unlisted Procedure due to the fact that there is not a description in the CPT for this procedure.

Hope this helps ;)
