We are looking into offering for our bladder cancer patients, Gemcitabine Bladder Instillation Syringe: 1g/50mL. Can anyone give any suggestions of a HCPCS code that we can use?

The documentation in the patient's record needs specify how much was given. The label of 1g/50ml is the strength, not the dosage given - the record should reflect how much was actually administered and/or wasted. So for example, if 50ml of this was used, then you'd bill for 1 gram, which is 5 units (1 g = 1000 mg = 5 x 200 mg).Thank you for your reply. Since J9201 reads 200mg and the label reads 1 gram how many units would i bill for?
