Recurrent major depressive disorder, in partial remission (F33.41), is documented in past medical history, and in the assessment, unspecified depression (F32.9) is mentioned. Current medications are Trazadone, Zoloft, and Klonopin.

For risk adjustment diagnosis purposes can we code F33.41 (given in past medical history) or we have to code F32.9 since it is mentioned in Assessment.
