United Healthcare all lines (commercial, Medicare Advantage, and Medicaid) has greatly increased the amount of medical records requests recently.
Is it common for a Medicaid company to request records after Medicare has paid as primary?

They can. Medicaid has different policies than Medicare, as Medicaid is state to state.

YES! It's insane!
We received a letter back from UHC denying everything we billed. The letter states the medical record doesn't have the physician's name.
The physician signed all the pages that she needed to.

This particular physician left our practice over a year ago.

How can I fight this denial?
