We are trying to figure out how to bill Medicare for a bilateral hardware block injection (L4-5). We have been billing it as unlisted, 64999, but it is denied.

Wiki Hardware Block injection in ASC

I used to work for an asc and this is very frustrating. We appealed every time with documentation and reports and suggested RVU's and expected payment to Medicare but bottom line, Medicare will not pay an unlisted code to an ASC - so we used to tell the doctors doing the procedure for hardware block that we could only do the case if another procedure was being done at the same time as the 64999. A lot of our pain doctors would do an epidural 62311 along with the hardware injection. I know this is frustrating, especially when you want to report the unlisted codes in hopes that eventually Medicare will provide an actual code for the hardware injection.

Yes, sorry this is unlisted and MCR will not reimburse for unlisted procedures
