You should look in your CPT book.. the 11200 indicates this code is for 1up thru 10 skin tags. If you notice in the HCPC look J2001 is a code for IV route which I doubt is how it was administered. You do not code for lidocaine given for any route other than IV. If the provider performed a skin tag removal at the same time as the annual exam then you would need a 25 modifier on the office visit. and you only bill 1 unit for up to 10 total. You will need an awesome diagnosis code to support the medical necessity for this.
