I need your thoughts on Group Therapy 90853. Do any of you require the diagnoses to be documented in the actual note? When you think of "each note has to stand on it's own" than I would say yes. RAC and PERM audits state as long as it's in the treatment plan or documented as the medical record as a whole, than no.
