If the Ophthalmologist documents that "gross visual field" exam was not performed due to patient age, do they get credit (a bullet point), since the exam wasn't performed but it was addressed?

I would say "yes"

Pediatric specialists and pediatricians are, unfortunately, stuck with the audit tools developed for a mature population.

If the bullet point is addressed and the reasonf for deferring the exam is noted, I give credit for it.

In general pediatrics the 1995 guidelines work much better. 1995 guidelines all for a "complete exam of a single organ system" to count as comprehensive ... but they don't define what this means.

I would recommend that your physicians spend some time outlining the expected parameters of a complete pediatric ophthalmalogic exam, add information on how many bullet points are required for complete vs detailed vs EPF and file those guidelines with your compliance plan.
