Dr. performed a graft 15732 and a layered closure 12053. Am I correct in saying that the 12053 is inclusive? Also he excised a BCC before the graft was done to close. Isn't the excision separately reimbursable?

Correct, per CPT guidelines, layered closure is included in the Flap code (this isn't a graft). The only thing that can be reported separately is if the donor site needs a skin graft or additional flap to close.

If you look under the MOHS guidelines in the CPT book, if repair is performed, use separate repair, flap or graft codes so the flap is not included in the excision procedure.
