I am writing from a CLIA lab with a coding question. We are runing a test that we will be using a misc CPT 84999. Can you please help me find a way to check if a Dx of V28.89 will require a report attached to the claim for billing?

The payer may ask after they review the claim. I have experienced this from time to time, but not as a general rule.
