Hello I'm looking for some guidance on proper coding for placement of a silo on a gastroschisis patient with planned delayed closure but with daily reduction of the silo. My general thinking is that the daily reductions would be included in the initial placement of the silo (49605) and then the secondary closure (49606-58) billed when they do close the patient. My providers want to know if they can bill daily returns to the OR with 49002. I do not this think is appropriate. Also, should 49605 be billed with a modifier -52 since no closure is taking place during that operative session.
