Wiki G0461 and G0462

Could I get some clarification, G0461 and G0462 replacing 88342 for Medicare claims beginning January 1st 2014, is this for only Medicare or does it also include Medicare replacement plans such as Humana medicare, Coventry, Medicare Complete just to name a few?

We have been using these codes for all Medicare plans including the ones you mention. In addition, these codes are needed for Medicaid and Tricare.
