Could someone please confirm if G0440 can be charged for each site if the cm are under 25? Thank you for your help.
 
Yes, according to our Apligraf rep, each wound w/G0440 abd an additional modifer of 59. If there's 2 wounds on the right leg and other are under 25sq cm, it would be coded:

G0440
G0440-59
