Wiki G0439 when a pt has a commercial ins as primary and Medicare as 2ndry

Hello,
We are having trouble getting paid for a G0439 when the pt has a commercial ins primary because they are still working or spouse is still working. When we submit it to Medicare after commercial denies it, Medicare states that another ins is primary and denies the G0439. How are some of you getting paid for this situation? Please advise and thank you for any imput.
