Our podiatrist has started billing the above to Medicare IL/MO as well as some commercial payers. Medicare is denying these as modifier missing/or invalid.

Does anybody have experience billing this code as well as a reference to the LCD on this?

I am not 100% certain of this, but it is my understanding that those codes can only be used by an MD as part of an office visit, not a podiatrist
