I billed Medicare for home health with G0180 and dx codes S71.002A, L08.9, B95.2 in this same order. Its very rare I receive a denial for my HH claims but this one got me. Medicare denied with information submitted does not support these many services. I believe that there have to be a mistake on the dx codes I'm billing or the order. Anyone knows why this denial? What does this denial means?
 
It could be a duplicate claim. If you accidentally billed them twice, and the first claim is processing when they receive the second one, you could get that rejection. Why it doesn't deny as duplicate, I do not know.

I've had this happen to me. I wait 10 days and then check claim status.

thank you. Tat makes sense. I just realized that I had this denial before on other claims and it was for been duplicate.
