Wiki G0008 and modifier 59

Does a modifier 59 need to be on G0008? Our facility had previously coded influenza vaccines this way and I do not believe it is correct. They would have a 99213, G0008-59, 90653. I think it should be 99213-25, G0008, 90653. Any help with this is appreciated!

Billing had also set up the venipuncture 36415 to pull 59 automatically and I do not believe this is correct either.
