Wiki Functional Limitation Reporting Question

This is a unique scenario:

Do I need to end reporting of the current primary functional limitation (G8984 & G8985) in the middle of the plan of care when the patient comes in for an evaluation for a seperate issue and discharged in the same day and we are reporting the same limitation set (G8984, G8985 & G8986). If so would we need to report the "continuation" of the initial plan of care as a "new" reporting limitation?

How do I report an evaluative procedure (discharged same day) when it is for the SAME functional limitation as the one that I am currently reporting (patient continues therapy for initial issue).

Any help/guidance would be greatly appreciated.

I would think that if it is the same functional deficit you would simply report it with the current status and goal status. If it would be a different functional deficit we are to report as though it is a one and done and report all three (current, discharge and goal). But if ongoing treatment and same deficit all that has been discharged it this particular problem but not the functional deficit.
