Wiki fracture coding help

can someone advise me on coding a closed reduction colles fracture that failed, and was immediately ORIF. DR believes a 22 mod should be added.

Mod 78-Unplanned Return to the Operating/Procedure Room by the Same Physician Following Initial Procedure for a Related Procedure During the Postoperative Period: It may be necessary to indicate that another procedure was performed during the postoperative period of the initial procedure (unplanned procedure following initial procedure). When this procedure is related to the first and requires the use of an operating or procedure room, it may be reported by adding modifier 78 to the related procedure.

Depends on whether this was done in 2 sessions or one. If actually two, then you can use modifier 78, but if they did a closed reduction in the OR and it just didn't hold so they proceeded to the open, then all you get is the ORIF. You code by how the procedure was finished.
