My physician had a pt present with a wrist fracture that he recommended be fixed surgically; however, the pt did not agree to surgery and wanted to see if it would heal with out surgery. The doctor performed manipulation, applied a splint, and told the patient to return in 1 week. When the patient returned they decided to proceed with the surgery. Closed treatment fracture care had been billed. Can he now bill for open treatment, too? It doesn't seem right to me that he would be paid for both. Any advise will be greatly appreciated!!!
 
You can bill for the open but a modifier is needed, I think it's -58
