I was hoping someone could help me with the coding and modifiers for the following procedures:
1. Left tibial plateua Schatzker IV open reduction and internal fixation
(fracture was reduced less than 30 days ago using external fixation, this was removed and it was re-reduced using internal fixation)
2. Left thigh and leg wound irrigation and debridement
3. Left thigh and leg wound delayed primary closure
4. Left leg wound VAC placement
5. Left leg and thigh hardware removal
For the fracture reduction, I got 27535 but wasn't sure if I should use modifier 78 or 58?
As far as the wounds, it seems like the closure would be bundled with the debridement, but since the closure is higher RVU's, would I only report that and not report the debridement?
The wound VAC would also be bundled with the debridement/closure, correct?
I do not do a lot of ortho coding and want to make sure that I am capturing everything I should be. I appreciate any insight!
