Wiki forgotten screw during removal

Patient had infected hardware in the ankle he so went in to have it removed. While in the recovery room the doctor realized he forgot a screw, so took patient back to OR to remove it. Should I bill 20680 twice with the 2nd 20680 with a 76? And what about a dx code? Any info would be great!! Thanks!

wow! you'll need to report 20680-78 for return trip to OR. Two separate surgeries reported as 2 separate claims. Expect the carrier to request note. Make sure doc puts in an 'indications' section in 2nd note to help explain. I would use same 996.xx dx code. Good luck.
