What code would you bill for a foraminotomy only during fusion?
For example in this op report for L2-S1 posterior lateral fusion he states: A laminectomy of L2, 3, 4, 5 was carried out. The central laminectomy was developed and then foraminotomies were carried out at 2-3, 3-4, 4-5 and L5-S1 with care being taken not to compromise the facet joints of L2-3, 3-4, 4-5 and L5-S1.
I am coding 63047 w/63048 x2 for the L2-5 laminectomies but I am unsure of the L5-S1 portion. Does this qualify for an additional 63048? Why or why not?

The foraminotomy is a component of the Laminotomy/ectomy.

This is what I would code:

laminectomies of L2-L5 (63047, 63048 X2)

laminotomy (hemilaminectomy which includes foraminotomy/ectomy and factetecomy/otomy) (63042) of S1.
