I need help coding this scenario. When it comes to allergies/allergic reactions, I'm not quite sure whether to code for them when the issue is resolved at the office visit.

History: Pt presents to office for followup of an ER visit due to an allergic reaction. It was felt that the new med he had just started (benazepril) might be cause.
Exam is normal.
Assessment: Allergic reaction, 995.3
Plan: Discontinue benazepril; add to allergy list. Avoid all ACE and ARBs. Start metoprolol.

But since the allergic reaction is not present at this office visit, I would not code for that, correct? (Visit to ER was 3 days prior.)

I'm thinking hypertension, 401.1, is the primary diagnosis...? Do I pick up any allergy code or history of allergy code?
