I can NOT find guidelines for medical necessity nor frequency on this code. It has a very high reimbursement rate, but I need to be firm in my answer to my physician on this; therefore, I need to have concrete info that I can not seem to find. Is anyone else billing this code and is it hard to get reimbursed for it? Thanks in advance for any help anyone can give. Kim
