If a patient is given 0.45 ml of Fluvirin, can you bill the 90658? (full dose is 0.5 ml)

Was this a single dose vial or multi-dose vial? If single, you can bill for the vial, as the rest would be wasted (thrown away). Check with your carrier for instructions on how they want that billed to them - some do require modifiers to show that there was wastage. The same for the pre-filled syringes.
