Can someone please tell me if this documentation is sufficient to code a flap (14040)?
Excisional debridement is performed using a scalpel and bone rongeur to remove the devitalized tissue from the periphery as well as at the base of the ulcer. The area is flushed with sterile saline solution and all the bleeders are ligated.
The plantar skin flap is then brought up dorsally and it is trimmed to fit. It should be noted that all the skin ends showed good, active bleeding. By using a #4-0 Polysorb suture, the subcutaneous tissues are then reapproximated in a horizontal mattress fashion and #4-0 nylon is used to reapproximate the skin ends.
