The flap was inspected. The distal one-third of the flap was completely devascularized. All of this necrotic tissue was debrided. The recipient bed was also debrided. It was debrided down to healthy tissue, as was the nasolabial flap. Once this was accomplished, the flap was inspected. Because we had lost so much tissue it was not able to reach completely to the columella. At this time, the cartilage graft is appropriately covered. Therefore, I inset the remainder of the flap at the base of the columella and extending to the portion of the inferior columella. She will require some healing by secondary intention before we decide to tape this flat down. I may attempt to use a portion of the skin from the nasolabial flap.

Would this be more along the lines of CPT 15630 and 11042?

To me, this sounds like 15004 although I have never had to code this for "revision" of a flap that was already placed once... Anyone else?
