Wiki First of 2-stage ACL reconstruction

The pt had ACL failure, status post reconstruction. Surgeon did an intraarticular debridement, removed hardware deep from femoral & tibial tunnels and performed bone grafting of both femoral & tibial bone tunnels. The surgeon mentions that 6-9 months down the road when the graft has incorporated the pt will return for the second stage of the ACL reconstruction. We billed 20680, 29870, 27599 & 27899. CDPHP is denying stating the op note does not support the unlisted codes. I don't think we can use 29888 because the the ACL was not repaired/augmented or reconstructed during this surgery? Any suggestions of a more appropriate code other than the unlisted ones that we used?

Our surgeons do the grafting as an open procedure, is yours performing them via arthroscopy or via an open approach?

Then I can get you the codes that they are using.
