Wiki Finger manipulation in office

I need some help with this one. The doctor did a finger manipulation (CPT 26340)(for a locked joint and an osteophyte) with a local block in the office. Medicare is denying this for invalid place of service. Does anyone have any suggestions?

According to my Ingenix coding companion, that procedure is performed under a
general anesthesia.
