Very confused as to what to do.a pt breaks her femur and goes to an ED. ED does an ORIF. the ED happens to be over 30 miles from her home. She comes to us for post op. Can we code fx care? Please help me!

Yes and No you need to check with the patient's insurance or the Treating ED and make sure that the ED coded the repair with a 54 modifer and then you could bill the surgical procedure with a 55 modifer as you are doing all the post op care.
Otherwise I think you are not going to get paid for anything depending on the insurance.

She is a self pay patient. Does that make a difference?

nope! You should code the same regardless if they have insurance or not.
