I'm hoping someone can help with the following op note. I cannot find the correct codes and would appreciate any help :)

Diagnosis:
Instability of the 2nd toe with crossover toe deformity and varus of the second toe

Procedure:
MP arthroplasty and a extensor digitorum brevus transfer/reconstruction of later collateral ligament to the 2nd hammertoe repair

Descrpition:
.....Attention was placed to the second toe where a longitudinal incision was made incorporating the MP and PIP joints. Transverse incision was made, arthrotomy was performed. The articular surface of the proximal phalanx was removed with a flat cut with a saw. A similar type cut was made on the articular surface of the middle phalanx. In regard to the third toe, the extensor brevistendon was dissected and released distal as possible. At that point the whipstich was placed at the end of the tendon. A hemostat allowed passage deep to the transverse metatarsal ligament. The tendon was passed from proximal to distal under the transverse metatarsal ligamint, between the second and third toes. At that point, a small drill hole from the Arthrex set, 2mm drill hole ws passed from at the origin of the lateral collateral ligamit at the proximal phalanx level. At this point the tendon was passed, advanced through the bone, and passed with a suture passer. It was then brought up, with the toe held in a neutral slight lateral position, the tendon was sutured to itself and to the extenson tendon with 2-0 Ethibond suture. Provisional K wire was used to pin the hammertoe, was then advanced across the MP joint to neutralize the forces on the tendon transfre collateral ligament reconstruction.......
