Wiki FAS Evaluation of 13yr old

A mother brought in her 13 yr old daughter into our clinic (Family Practice) for a FAS evaluation. This is the first time that the possibility of FAS has been mentioned. The mother feels that it is in her daughters best intrest to behonest about the prenatal exposure to alcohol. The daughter's schoolwork is declining. She has a poor attention span, is irritable and has temper issues. Our medical prodiver is referring the daughter to Behavior Health Services for a "true" FAS evaluation.

Our provider has not given the dx of FAS. The documentation states " +prenatal ETOH exposure". Any suggestions for a dx code to take back to the provider?
