should this modifier be used when a patient receives vaccines, rapid strep testing, accu checks, glucose (A1C), UA's, etc? This is pertaining to the facility charge only; ie 99213

If any of the above are done by the nurse, MA, or CNA, should there also be a modifier added to the physician charge? Thank you in advance for your assistance.;)
 
The 25 modifier is used on the facility charge anytime a significant procedure with a status S or T is performed. If your facility software does not indicate the status of a particular CPT code then you will need to look it up, these are listed in numerous resourses such as addendum B of the federal register.
If these are performed in the facility by the RN or MA then these procedures do not appear on the physician claim so there would be no need for a modifier on the provider side.
