Wiki Facility 36000 Charge

When billing the CPT 36000 in the ER setting for the facility, would you use Revenue Code 760 (General) or 761 (Treatment Room)? I ask because some payers reimburse differently depending on which revenue code is used and I can't seem to find any guidance on which is correct. BTW- the 760 code is paying higher

Wouldn't it be 0450 when done in the ER?

You would think, but the payor BCBS does not allow that CPT/Rev code combo.
